does anyone know the difference between the Sensar 111 and the Sensar 1V? is it just the sidewing?

The Sensar III is the typical Batwing amplified antenna. The Sensar IV is the same antenna with the 'Wingman' UHF antenna attached as one unit.

Check to make sure you have room on your roof for the Wingman attachment when you crank down the antenna. If you do, and you are considering a replacement antenna, then go with the Sensar IV, as the prices are nearly the same between the 2 antennas.
