Amber turn signal lens

I Have a 2000 HR Admiral that has cracked front turn signal lenses. I have tried to Google the numbers on the lenses but can't find any info on them.
Someone said it is from a Chevrolet Blazer. I can not find anything and any help is appreciated.

RV DAN ,you might try nwrv supply in ORE. they bought up all of monaco`s inventory when they change hands. They show some stuff on their website but is best to call them. Hope that helps
