TSM, many thanks for sharing your experience and pics. Never heard of a freeze plug failing from the outside in but what you shared makes sense. I'm going to check the ones I can see as a result of your post.

We had a similar problem caused by a curled paint chip holding dirt on a freeze plug. Ours was behind the exhaust manifold. The leak looked like a head gasket leak. We were glad it was the manifold that had to come off and not the head. Sadly getting the manifold off also resulted in 6 of 12 bolts breaking. We replaced all freeze plugs in that area, not just the rusted one.

Just curious, why you did not install a brass freeze plug?

I ordered the replacement from Cummins to ensure a proper fit. It is smaller than a typical automotive plug.

Found the invoice. The Cummins part number for the expansion plug is 143066 for a grand total of $3.19.
