I would check also the air filter. See if it's clogged or damaged.
Does the turbo spin up? Can you hear it?
A trip to a Cummins shop might be needed to find out what the fault codes are. You may also have lift pump or injection pump issues, fuel starvation, etc.

I had the same problem and it turned out to be a turbo. Not cheap to replace . warranty cover about 2/3 of it.

My freightliner xc chasiss has a fuel strainer in it before the filter, My symptoms were not nearly as severe as yours but I was getting check engine under heavy load. After replacing fuel filter and pump it turned out to be a plugged strainer, $15.00. It's a little tough to find but if you have one I would definately change it first.

Fuel starvation, was the probable cause of injector pump failure, as the injector pump uses 70% of the fuel it gets from the tank to cool and lubericate its self, then returns it to the tank. So by the time you use a tank of fuel in a diesel engine it's all been through the filter about 5 times. Thats why they plug up so fast. Running lean and probably hot from lack of fuel, more than likely took out the head gasket. Unfortunately for you; learning this the hard way; knowing the location and having the filters both changed regularly, may have prevented both problems.

I have a 2004 freightliner xc chassis. The fuel strainer is in the loom on top of the frame passenger side behind the rear wheels. It's hard to get to, I was able to squeeze in after fully extending the rear jacks but I'm not a real big guy. You'll likely have to cut a couple of zaps to find it amongst all the other wiring and lines. Once you're there and have found it, it's pretty easy to change, have some kind of a pail to catch the diesel that will drain out.

What engine do you have? I am having the very same problem, with an ISC 350. Freightliner says I only have one filter. I agree that all of the wiring & hoses are hard to look around. Please give me a reply, as mine is now sitting in the shop!
Thanks!
