No TV antenna or cable TV selector button
on the outdoor TV?
Four televisions in this unit and a switch box for only 3.
How do I set the outside TV for cable?

It's probably split off of one of the others. Just turn tv on and switch through the other TVA until it comes on.

It's tied in with the one over the dash.

I have all three buttons on switchbox set to cable and receive cable on all 3 interior TV's but no cable on the exterior
....

If you do not have 4 tv selectors then it's either a prior owner mod or it's split off one of the others or both.. OR you are seeing a connection and ASSUMING it is an output when it's an input..
Only way I know of to find out is to push every button and test.

Yup, check what Bullheaded suggested. Our outside tv is tied to the bedroom tv.

It is wired to the bedroom TV on my unit. I disconnected the switchbox when I was using the satellite on the coach to provide TV for my house, after Hurricane Sandy, and connected it back incorrectly..
Duh!
