This is the original lube plate, which was copied about 8 years ago by some major hitch mfgrs. It is also the only 12"D disk I know of that's sold. Reese and other 5er hitch mfgrs recommend 12" disks for heavy 5ers. I've broken many of the cheaper 10" copies sold by RV accessory stores.

What's the name of the company? Do they have a website?

I think we have the 10" disk, and so far...for over a year...haven't had any problems with it (knock on wood).

The website is for a different product. Since this is (apparently) a very small company, a phone call is best. The 12" disk may be found/ordered by some RV supply stores too.
Even Reese, who allegedly copied the idea 8 years ago, does not offer a 12" disk; yet they recommend a 12" disk for heavy 5er's. The 12" disk is stamped out in one piece, other brands are injection molded in three pieces, which is why they often break. Our 5er goes through a 10" about every 5K miles.

i stil have my original lube plate 12" i bought in 1996
its been on three fivers and still looks and works great.
