Not sure I fully understood your request, but this is what I use when I had to pull a tow dolly and also have the bike rack attached.

We now tow 4 down and have a receiver on the Jeep which holds the bike rack. Plenty of storage inside your tow car while your traveling.

Contraption below is available at Northern Tool.

The Bike carriers (Two kinds one mounts under a ball of a ball hitch, one you described) do not extend the hitch at all. ANYTHING that extends the hitch, including extension devices, drop (riser) hitches and so on will reduce the hitch's capacity.

What is a 5,000 pound hitch with no extension can quickly become a 4,000, 3,000 or even 1,000 hitch when you drop, raise, or extend the hitch.

That said.. I'd use like a 1 foot rise/straight through hitch and put a standard cargo carrier on the upper hitch.

I used almost this same double tube last year on a number of trips. But I caution that, in my case, I was towing a very light (1600 pound) sand rail TOAD four down, and I only had one light weight bike on the rack.

Carefully review all your weights when you do this. Better to be without a bike than lose anything off the back eh?
