I use a Wilson trucker antenna and a Wilson boost amplifier. The amp has a small flat antenna that velcros to either the MIFI or a cell phone. It will increase an existing signal. There is nothing out there that will guarantee universal cell phone, MIFI, or aircard coverage though.
You can see the amp HERE

Hi GeoPen,
I have had a 3 WATT booster with the antenna on the roof. I get service if the antenna can see a tower. There are places where the terrain makes this impossible or there just is no tower between me and the horizon. There are not many places where I do not get service (AT&T) but there are a few. Along RT 321 on the West side of the Great Smokey Mountains, in Tennessee. Many places in Montana and Colorado are a couple of examples.
The signal meter on the phone is the signal one gets from the tower. The tower transmits at 45 WATTS of power. The tiny phones carried by most users are around 1/4 WATT of power. This means the tower may get to you, but the phone getting back to the tower is another matter.

I use a Wilson amp similar to what Clay described. I have the antenna inside the coach in a high window above the front passenger seat. So far it has worked great there. As stated, there does have to be a signal to be amplified ...they won't create a signal where there is none...

I think you, as I do, will get the most bang for your buck from a Wilson model 811210+magnetic trucker antenna. This costs about $200, including 12V cigar plug, and it will work for both your cell phone and plug in directly to your laptop(says Wilson owners manual) for signal amplification with optional USB adapter. I bought mine from 811210 - Wilson Cellular SIGNALBOOST HDSOLO Mobile and Home/Office Cellular/PCS Dual-Band Amplifier
The magnetic trucker antenna and 120VAC adapter are extra.

Thanks to all for your help. We are going to get
Wilson but don't know yet which one. We found a Wilson 841245 wireless for under $400.00 new. Looks like a good deal.
Thanks again
