Has anyone used the Girard? Do these things really work as advertised? I know good water flow is critical for them to work properly. We are using our 5th wheel only in one RV park and the water flow and pressure here is consistently good.

IMO, given where you plan to use your RV, any downside to the tankless heater will be mitigated. There are, generally, two criticisms of the tankless heaters in RVs. First, it usually takes a couple gallons of water to get them going for the first time as they don't fire-up until flow is sensed. For boon dockers, that is an unnecessary waste of precious water, though it can be saved in a container and reused.
The second issue is the flow rate cut-off. Most tankless heaters will shut down the burner if the water flow falls below ~1.5 gpm or so. If using hot water and mixing it with cold and the hot flow drops, the heater will shut off and you've got just cold water.
From your post, it doesn't appear that the "waste" water issue will bother you, but you need to research the cut-off flow of whatever heater you choose and, if possible, check to see what your present hot water flow rate is when in typical use, as in a shower.
