Not that I have found. Every source I have looked at does not cover all of them.

Allstays has a ton of them. Not sure it has all, but it's got to be most of them.

The Good Sam RV Travel Guide is about as close as you will find. There is a map for every state section. Accuracy depends on what people report. It includes all campgrounds, not just Good Sam participating campgrounds. It's about 2 1/2 inches thick and sells for $10 if you have a Good Sam membership. It does not include state parks, Corps of Engineers campgrounds on any of the other government sites. All Stays will cover most everything (truck stops, dump sites, etc) if you want to go that route.
An all inclusive map/directory would be a tall order and be out of date as soon as it was published.

We've been using We Camp Here ( We Camp Here | Where happy camping begins.) for the last few years. It has a fairly extensive listing of campgrounds with quite a bit of information. Not too many reviews as yet, but getting better as it gains popularity.

IMO that would be imposable, (and pretty much useless).
See this map showing all campgrounds and rv parks in Texas.
All Texas RV Parks and Camping Map
Now imagine what a US and Canada map with all "campgrounds and rv parks" on it would look like.

No; reason_ some small CG's do not advertise-nada, not anywhere. There's one about 35 miles from our house that way. It's a seasonal CG, normally supporting the National Muzzleloaders Assn shoot every year at Friendship, IN.
Usually each state has a CG owners assn. website listing most smaller ones.

As others have noted, the easy answer is no. My solution has been to use AlStays Camp & RV app, combined with RV Park Reviews to get an idea of what users think of them. I also have used the Ultimate US Public Campground Project website and my GPS campground find, however the Garmin has sent me to many trailer parks.
