Most states west of the Mississippi now allow for a 14 foot max height and a 8 foot six inch max width. But back east 13-6 is pretty much the norm. So go 14 foot and you can clear any legal vehicle in any state.

Our RV garage is a pole-type, 60' deep, 40' wide, with useable 14' high X 20' wide sliding door, 6" concrete floor, and a loft opposite the RV side. You may get a lot of ideas from this website:http://www.ag.ndsu.edu/extension-aben/buildingplans/ None of the building plans directly address an RV garage, since this website is dedicated to farm buildings, however It was great for ideas when I built our RV garage.
