Rally Data Request??

We would like to request a "Rally Data Request",don't remember getting one in an email,so how do we get one to fill out?? Thanks in advance!

I can only work on the rally data on weekends. I hope to go through the registrations this weekend and see who all has not sent in the data for the rally.
