I saw something similar in WV this past spring. A couple was pulling a rather large 5th wheel behind a single axle Volvo tractor. Had a sideways ramp arrangement set up to carry a smart car between the cab of the tractor and the front of the 5th wheel. I wanted to get some pictures but didn't want to seem imposing on him and his wife. Only issue I was was that is is a long way down from the platform to the ground.

I remember the driver was a mountain of a man with a big cowboy hat and the Volvo was gold in color. Very nice looking outfit

This is a very common way to carry a car ( smart car is perfect as it fits sideways and meets fed width stds ) on a converted tractor. There are several ramp styles and hydraulic lift methods to easily move or winch the car on and off. As a former truck driver , I looked into this when I had a fifth wheel but decided to go the Motorhome route.
The op photo however is unique to be sure. I'm sure the Motorhome has no stress on the rear frame and will do just fine. ( not) It is a tractor based motorhome so at least it's offsetting the front engine however , good you took photos and moved around quickly !

All it takes is money...lots and lots of it!!
or
BUT...the Smart ForTwo in North America is 106.1" total length, so actually too wide to ride that aspect without being a WIDE LOAD on US roads.

Not the one I had in mind but it gets the point accross. I don't spend much time watching people set a site up or watching them go but like you mentioned in your blog he's one I would have watched.
