As builts are only as good as your record keeping! We had a crew doing demo for a total remodel of a Dental Clinic and all drawings were consulted back to 1953 originals. Main circuit panel was supposed to be completely dead but they didn't check it twice and cut a line. 2 480 lines were still live and luckily the arc flash didn't kill him but did blow the worker about 15 feet back. Even the electric company didn't know the lines were there and that accounted for 30 years of free electricity for every business that ever had the building. Lock out tag out works if you are positive all lines are deenergized.

Your right about checking it twice. That's something you always do before working on a piece of equipment. So he was working on a delta system?

Could be worse... When I was in college they were building a new "Fine and Performing Arts" building, The workman digging the foundation sliced through one of the high tension lines with his backhoe, he's ok, rig took the shock just fine, But this was one leg of the 3-phase primary to the school.

Well, the main transformer that fed power to the entire campus tried valieltly to continue to do so with only two of the 3 legs but it could not, it quickly over heated and That was that.

Then (As the lights went out in Battle Creek) the school found that THEY, not consumer's power, owned that transformer, and since they thought it belonged to CP, knot KCC, they had not insured it (OUCH).

I assisted maintenance in getting one poor student trapped in a no-ground access floor, in his electric wheel chair, out of the building safely. (easier than it sounds, Jack elevators were on a higher floor, we opened the doors, opened the valve, lowered it just like a bottle jack, closed the valve, let him in closed the doors, went down stairs to a ground access floor (This school has lots of ground floors) and repeated the valve thing, took him out and job done.

A couple years later the same Maintenance chief I worked with getting my fellow student out had gotten a new job, Teaching at a new vocational school.

A workman digging the foundation to a new building sliced one of THEIR 3-phase legs feeding the transfoermer that fed the school..

The teacher confronted a janitor and ask him to unlock the transformer room so he could pull the main switch and save the transformer.. Janitor balked, Teacher forced the issue, some time later when the building electrician arrived and saw the temp reading on the transformer (high) and the open switch he said "Who turned this off" teacher said "I did" Electrican said "THANKS! DO you know how much you sved us?" Teacher, as it happens, did know.. As he'd been there and .. done that.

They hd to remove the roof of the power house to replace that transformer at the college.

Some of those old RV parks have no drawings. They were made into RV parks before all the codes were set.

Doesn't matter about "codes", any contract should include providing as built plans to the owner.
