Cable tidy door?

The cable tidy door on our old rig has seen better days. I've been looking for an exact replacement online but I haven't found one so far. From what I can figure out, there is supposed to be a shutter type door which slides down to close off the small opening when the cable is tucked away. I guess I could make something out of plastic if I knew what it's supposed to look like.

Does anyone have a photo of theirs so I can make one "true to form"?

Also is there supposed to be a warning sticker on the door in the 3"x2" indent just above the opening?

Thanks guys. Yes I spotted that one on ebay. It's similar, but not quite identical. I'm guessing the EXACT 'cable compartment door' is no longer available....
