2003 coachmen mirada slide problem

Just returned for our maiden voyage, and slide is now inoperable. It appears that the bolt holding a rod that the scissor mechanism on the forward side came loose and when I extended the slide, it bent the bracket. I then attempted to retract the slide, and it pulled the top 1/3 of the bracket out of the wall...wondering if anyone else has had this or a similar problem...remanufacturing the bracket will not be a problem, but I am looking for advice/knowledge on how to reattach the bracket to the coach.? There are 3 attachment pints, and they have rounded over heads, ie no way to loosen or remove. The top one appears to have sheared off...help!!!
I have attached a pic of the normal assembly on the rear side... Bolts/ rivets are under the vertical pin...
