You could try cleaning the MAF sensor. If that doesn't clear it up, replace it. The rough idling and other issues are classic MAF sensor symptoms.

Hi CMB36 and Welcome to the forum!
You say you checked the fuel pressure and replaced the regulator. Was the pressure reading off that prompted you to replace the regulator? Did you check the pressure again after replacing the regulator?
I agree that it still sounds fuel related.

The MAP sensor (manifold absolute pressure sensor) is usually in the intake manifold of throttle body injected engines. A MAF sensor (mass air flow sensor) is in the air filter housing or the tube connecting to the manifold. They both help the ECU calculate fuel/air mixture.

They still need cleaning, and if dirty will cause the issues you report. If cleaning doesn't improve it, then replacing it would be next.

Is your 1995 460 equipped with a catalytic converter? I ask because my 1994 isn't. If it is you may have a failing catalytic converter clogging up the exhaust.
