bdozer, thanks for the pictures and sorry for your problems.
From what I can make out of your kitchen slide and the grooves in wood or metals can only suggest right now is look at the very end of this spec and the pictures of shoulder bolts that your may have missing or loose that are suppose to guide the slide plate and slide true in its opening.
This new slide thread is where the spec has come from with many updates and all this you will be able to find in the Important Information sticky for Newmar Owners.
After getting familiar with these slides maybe we can fix some of your problems.
May want to check the SLS Method of fastening motor bolts for bedroom motor,,

Well thank you ka5gyo I new there was a year of chain driven kitchen slides but didn't remember the year or time. 2011 DSDP 4324
For a 2011 coach the area looks as clean as a new coach, so I will assume you have never had a problem with the chain drive of the metal plates, is that correct?

Correct for the most part.. Have found loose bolts, red locktite did not help , so I have drilled and tie-wired the bolts. One chain got slack and skipped a couple teeth. I just slipped it back to proper position and tightened the chain. Other than that , all is good.

KA5 were the loose bolts the shoulder bolts for pan or your motor bolts you are talking about, have found most problems with the kitchen slides have started with the loose shoulder bolts and slide out of alignment in its opening.

I would describe as the gearbox mounting bolts to bracket with slotted holes have become loose most often. This is where the most torque is involved. Slotted holes are great for alignment purposes, but will work loose. I have actually tighten all the bolts several times, all around, even the carrier bearing bolts, with slotted holes also. Not sure what you are describing as shoulder bolts though.

Took a while to figure what this gentleman was talking about but turned out to be a kitchen slide and loose shoulder bolts.
This link will show shoulder bolts he had to adjust. some other kitchen spec's show larger bolts and some smaller in the new slide thread.
Shoulder bolts have been the cause of many's problems that's why I wrote the new thread.

Thanks 007...great work. I can only add that the slide motor bolts should be checked when they are not under a load. In other words check them and tighten them after you extend the slide and bring it back in a couple of inches. I am sure it has been discussed but is worth mentioning again as it may contribute to bolts getting loose after being tightened.
