Power, Overheating

Our 2003 Dutchstar (CAT-e3126) with 127k miles doesn't seem to have the power it used to and doesn't stay as cool. I wonder if the cause is the turbocharger. Should I have it replaced? How much might it cost and best place to do it (we live in western Washington)? I usually do all the work on it myself, but at 75 it probable isn't something I should undertake

Change a turbo because of mileage-- NO. If low boost and no leaks in the exhaust or intake side, sure. But only after ruling out leaks.

Diagnose the issue and fix that.

And/or verify that the Preventive Maintenance has been done. For example your first VALVE ADJUSTMENT.

YES, do check for leaks in the exhaust FIRST. My 8.3 Cummins lost power & was shifting down a lot. Come to find the exhaust manifold was cracked & some small pieces were missing. Use a shop vac's exhaust to charge the exhaust system on your unit & go looking for leaks between the head to the turbo charger. I ended up with a new rebuilt engine. I am your age too. I have always taken care of my autos, boats, Cessna, & now RV, but this job was to heavy for me to do. John

Plugged fuel filter or air filter could cause low power. When was the last time the filters were changed?
