Slide Out Floor (Subfloor?)

Found an issue with the slide out. When we folders out the couch to make a bed we noticed it "teeter-tottered" quite a bit. After looking under the couch the bolts closest to the wall looked like the nuts had backed off letting the couch rock. Once I got back there with some tools I noticed the bolts were able to move around a lot. It looks like the wood flooring under the couch has rotted out and the bolts have pulled up and almost out of the floor. I couldn't find anyone else having had this issue before. I plan to pull the couch out and remove as much of the bad wood out as possible and try to rebuild it. Any information or insight to the project would be appreciated.

I have a few specific questions about the issue:

1) Were the original bolts accessible from the bottom of the slide?

2) The couch seems to be raised slightly. Was the flooring thicker here?

3) Is the dinette built on the same sheet of plywood?
