What kind of rig?
This happened to us once, the engine finally stopped running requiring a tow. We thought it was a bad load of fuel also. Turned out it was the fuel pump going bad. Replaced it and all was better. Might check your filters also.

Sure sounds like fuel starvation to me. The fuel filter is the first thing to check. Or just replace it, cause that's easier and more effective. Next would be fuel pump(s).
Premium does nothing for an engine designed to run on regular grade gasoline.

Any codes coming up?
Sort of sounds like a bad MAF sensor or circuit.
I second changing the fuel filter.
I have a manual fuel pressure gauge to monitor it as I drive. Great little addition to have.

What brand gas? I got a bad slug of Flying J once in AMA and it started acting funny with about 18 gallons left in the tank. Filled up and it ran like a top ever since. Probably too much ethanol or water.
