FWS Lock stopped?

While setting up yesterday the DW was deploying the FWS and we both heard a loud noise which prompted her to stop and I investigated to find the rear lock had stopped half way in and slide had started to move causing it to jam against framework. I had her bring back in and lock, then retracted again and all was OK except the lock pad has now broken.
Now to the questions:
1. What would cause the system to not let the lock retract before allowing the slide to move out?
2. Is there any reset I should do to test the system operation?
3. Where can I get replacement parts for the lock motor?

So you think the lock came loose?
Have you opened cabinet wall to inspect lock?

Everything appears to be tight and normal other than the broken tip on the Latch.
