The dealer will (or should) know of the recall. If they don't, you can inform them of the recall number when you call for an appointment. They will inspect it, make any repairs if necessary, fill out the card & either mail it in or give it back to you to mail in.
At least that's how it worked when I had the Norcold black box installed years ago.
