Does the plate / cover look like this?

If so the last I knew there was no supplier anymore.

But this website has some 3d printer produced covers although the labels may not be available through them.

Call REV Group, they usually carry those type items.

I have the same covers and found them on eBay. Don't get the non original dual plate cover. They are 3D printed and look terrible. Here's a helpful hint. buy the singel covers they fit perfectly side by side. I've replaced all of mine. https://www.ebay.com/sch/sis.html?_n...=114206879309&

I ordered new switch covers from American Technology Components last year - 1-800-238-2687. They will need you to take off the front plate and give them the part number and what labels you need. They were very helpful.

Does anyone have a source for the switches? I have one in my 2004 Revolution that needs replacing.

I called REV group a couple years ago and they sent me a whole sheet of the new labels for the switch plates.

Peel and stick raised gel type/looking labels. Give them a call.
