Check with Source Engineering. These guys used to work at the Monaco diesel manufacturing plant. They should know where your shaft was made and possibly where to find a new one.

But any decent driveline shop would be able to make it from parts on the shelf. My guess is that it would be more expensive than the one above but doesn't hurt to ask.
