Engine has a roar that comes and goes at highway speed, what the hell is it

When the thermal hub in the rad fan gets air at a certain temperature coming through the rad , it ramps up to full RPM to aid the cooling.

Does it occur when hot outside and you are climbing a grade?

Probably normal, cooling fan kicking in.

It sounds like a jet airplane engine, nothing is wrong. The system is working as designed. Scared the hell out of me the 1st time it happened.

Installing a 180 degree thermostat helps with the noise. It doesn't eliminte it, just doesn't roar as often, or as loud and long as it does when the engine is equipped with the stock 195 degree thermostat.

I would NOT recommend changing to a lower temperature thermostat. This engine with all it's sensors and a computer is designed to run at the 195 deg. level. Running too cool can and will lead to poor mileage, engine sludge build up, carbon build up and possible catalytic converter failure.
Once you know what causes the roar you should rest assured everything is working as it was designed to.

After running a 180 degree thermostat in my 8.1 Workhorse for over seven years, I can state with confidence that none of the anticipated problems you list in your post above have occurred.
The only downside I have observed is lower cab heater output temperature.

I don't find the the so called roar that bad. After running up and down some 7* grades on ark. 65 it only kicks in for maybe 60sec. I like knowing that is working and that is what it is deigned to do. The temp. Never changes. Now when it doesn't kick in then it will bother me.

I agree that a lower temp thermostat is not a good idea. The computer is calibrated for a higher operating temp and the engine is most efficient at that temp. It affects the air/fuel ratio and also the catalytic converters. Just because you did not notice anything does not mean these things did not happen, just means you did not notice them. Also known as anecdotal evidence and is the lowest form of data scientifically.

Lots of engineers design engines these days and they all agree on 195 degree thermostats.

For the OP...just so it is written...

The cooling fan on the front of the engine is always turning. But at speed, the fan hub lets it slip and is therefore quieter. When demanded by engine temp, the fan hub locks and turns the fan at full speed of the engine (by way of the serpentine belt) which is louder due to the high capacity of the fan.

That's the way it is supposed to work and reduces the need for a supplemental electric fan sometimes seen on the front of other truck radiators.

The first time mine did it, I knew right away what it was. To me it wasn't loud at all because when the fan on my old 1992 Bounder kicked on it sounded like you were inside of a jet engine
