Beauty Cap Failure and potential hazard

I have now had two identical weld failures of the bracket that the retainer bolt
screws into for holding the Beauty Cap on the wheel. The first failure occurred
a few years ago. I heard a noise and saw the cap leave the wheel with a lot of
rotational energy. Fortunately I was in the passing lane and it was the left tag
wheel. Had a car been there it could have been ugly. I bought a new cap,
bracket, and retaining bolt from Freightliner for $185 and chalked it up to a bad weld. Then last month after a trip from NY to FL the same cap was missing on the same wheel with the welds broken in the same way. Never heard or saw that
one go. I called Freightliner and they claim they never heard of this being a problem and had no real suggestions as to root cause. I can not see why this wheel is any different than the other three in terms of stress. Any thoughts on what could be causing this? I would not put another stock bracket on that wheel without reinforcing it in some way.

I removed the hubcaps from my motorhome to save the weight. They also made noise going do the road so now no noise and lost around 75 pounds.

Yes, I guess that is one solution but I like to try and figure out the root cause if I can. If no one else has had this problem and I have had it twice that leads me believe something is going on with that wheel. Those welds were stressed to failure from the rotational forces. The only thing I can think of is that hub cap is
slightly off center increasing the rotational force.
