Yes. The two small couplers on the sway bar are loose fit over the two small balls. That's not a problem.

You're confusing your apples with your oranges. The "handle" is the on/off handle. That is the one you turn clockwise until the threads bottom out. The brake tension screw is that other bolt head below the handle. There's no handle on that one - you have to use a wrench. The tighter you tighten that bolt, the more friction you create for the turning hitch to have to overcome before the rig can turn. You want it good and snug, but not so tight that the sway bar cannot slide within the case.

Yep, it's not a tight-fitting assembly. The cotter pins are simply to prevent the coupler from falling of the ball. There's very little pressure on those keeper pins, so they can be loosy goosy.
