Failure of WD Chain Hookup

Has anyone ever had a failure of the tang you put the breaker bar on? Not where the chain attaches, but the tang on top you use to snap up the chain into place.

Note in the attached picture, there was a flaw as you can see from the rust intrusion. Just curious due to age if there was an issue at one time, or do they wear out eventually.

FWIW no one was hurt. It snapped as I was undoing the chain, and just scared the wits out of me as the tension bar was thrown down.

When I had a TT I always raised the tongue of the trailer (using the electric tongue jack) high enough so I didn't have to put a strain on the "tang". I could install and remove the chains by hand. The pic does look like it has a little rust in the metal but I've never heard of that kind of failure.

That to me looks like a bad piece of metal. There is paint present
