Exact same thing happened to my mother, and it was a big brand, new car Dealership that did the oil change.

Sometimes things just happen. I have a buddy who works for a shop. One time he forgot to put the oil cap back on a car. Fortunately all he had to do was a major clean up. Now he always puts the cap in the way of the hood latch, so he can't forget it, and no one else could close the hood up without noticing. It's something I now do on my own cars as well.

It is always a good idea to check things or watch.
