duo therm 76000 series furnace problem

My furnace heats up for about 5 min then the blower turns on for about 30sec then turns off for about 45sec the cycle repeats itself as log as it takes to reach temp on thermostadt the thermostadt disinguages the flame stops the blower stops, but then the blower cycles one last time before the blower would run constantly till the room temp was met. not it gos on and off and the heat is not very strong. any suggestions
