I wouldn't try to fix it. It's 16 years old and even if you find parts they are going to be expensive.

I would replace the faucet with a quality one such as a Delta or Moen. A 2005 model most likely has pex plumbing and no cutoff valves. You can purchase those and the faucet at Lowes or HD. You can purchase SharkBite valves (cutoffs) that require no tools or glue for installation.

The faucet will have either one or two nuts holding it under the sink. You'll have to contort to get into the sink cabinet to get to them. Hopefully they will not be rusted so they will come off easy.

Most kitchen faucets are either single hole or 8" center to center of the handles.

Good luck!

Toss it and go to Lowes or Home Depot and buy a good replacement. I just replaced mine in the kitchen and the cost was $200. You can get less expensive but I liked this one and it has a pull down head. They are not hard to replace except for the possible contortions you may have to do to remove it. Like mentioned earlier there will not be any shutoff valves in the water lines but I don't consider that a big deal. Just turn off the water outside.
