If you had clogs in your filters, I wouldn't think it would run nicely for 15 minutes, then start acting up.
I had a clogged filter when the soy diesel mixes started coming out in the early 2000's. No one apparently tested to see what happened to that stuff when it got cold. Once the filter was clogged, it did not run good for a while, then run poorly. It was just done. It wouldn't die, but the fuel flow was so low it just had no power at all.
While not in a diesel, I had a fuel pump once that provided full pressure until it heated up, then it would act up which caused the engine to start choking and eventually die. The fact that you get apparently good fuel pressure for so long a time and then it goes bad makes me wonder.
Not saying it's not clogged filters or gunk in the tank as others have suggested. Just throwing out another thing that could be a potential cause.
