Could have been bad.
The photos show the towing fitment that was attached to the front cross frame on a 2018 Jeep Wrangler. Notice the holes that the bolts pulled through. Luckily this fell off in a service station parking lot, not on the road. Visiting with a service center, I was informed that the only safe way is to mount to the frame, not a frame brace as I was instructed by the manufacturer. This could have been far worse. Please do not mount tow fitment to a light weight frame brace.

Need to post manufacture of tow bar and picture of bad directions so people can check so they don’t lose their toad. Looks like it was just bolted to sheet metal.

Tow bracket is always bolted to the frame no matter what type of vehicle, unless it is a tow rated bumper. Everything else will tear off over time. One question is where was the safety cables that go around the frame?

This brings up a good point about inspecting base plates, Roadmaster for one states that the bolts must be inspected for correct torque every 3,000 miles. Which is something I suspect no one does given that the front bumper cover must come off on most cars to get to those bolts. I am getting ready to pull the front bumper cover on the cheap Toyota Yaris TOAD I just bought because I suspect it has not been off since the base plate was installed 15 years and over 200,000 miles ago. Should be interesting to see.

Wow. That is a Currie RockJock tow skid plate/tow bar mount. That is what I used to tow my 2016 Wrangler with before I traded it for a newer Jeep. Luckily, I never had a problem.
I'm not saying this was installed incorrectly, but I've installed all my tow set ups myself. I always recommend that if you have the ability, install yourself. Or, at least supervise with knowledge of the installation instructions.
