In terms of RV specific things, in my experience -- usually it's because the city water / tank fill valve didn't close all the way, allowing fresh water tank to over-fill. (Leak comes from a drain tube, as designed).
Holding tanks in general are prone to leaking when overfilled, it strains the inlet connection. Always a fun day when that happens at the black tank. When I first started RVing, for whatever reason I had this idea in my head to let the tanks fill before I emptied em. Better to empy those things very frequently, for several reasons.
Once it was a cracked strainer (just downstream from the pump). Probably freeze damage; if you camp in cold weather, on some rigs you need a heater in the wet bay (a hundred watt incandescent bulb may be enough). Twice it was a bad check valve, which caused water to shoot out of the faucet in the wet bay whenever the water pump came on. Not uncommon for those to fail. Typically just downstream from the fresh water inlet, replacements available at home depot.
