Well, I think I found the problem. The pulley that was in the sealed Motorcraft bag had 1L2E-19A216-AC stamped on it even though the bag had the correct part number.

No, all the belt is not on the pulley. It could be installed backwards, I'll check that next, but I installed it just like the idler pulley that was originally installed. The pulley is the same...

I just tried the install of this pulley (1L2Z-8678-AD) and it doesn't appear to line up. Does this look correct that only about 2/3s of the belt is riding on the pulley? This is on a 2017 E-450.

I'm going to be doing this mod this weekend. Just to clarify, the pulley goes up against the block, then the washer and bolt go in, right? Nothing between the pulley and the block?
